Here’s what Andrew’s morning looks like from this perspective—a view that is an important start toward understanding what OCD really looks like: Andrew trotted down the stairs to the kitchen, carefully avoiding touching the banisters with his hands or brushing against them by staying right in the middle of the steps, because he knew OCD would make him feel really bad if he got near anything that could be dusty or dirty. The first thing he saw when he got to the table was that his mother had put out a spoon from the extra silverware, not the usual stuff.

Trying to help her son, Andrew’s mom made sure everything was clean and made sure Andrew knew everything was clean by keeping the breakfast routine consistent. Unfortunately, that only meant that when OCD’s expectations were not met—even in a tiny way like using a different spoon—the fear of contamination would naturally be triggered. If everything was the way it was “supposed to be,” he would stay germ free. The minute the tableau was disrupted, that guarantee was threatened. So what kinds of things can trigger OCD?

For example, there are many possible triggers for OCD that are not visible to an outside observer. In one, the child’s obsessions and compulsions aren’t set off by a reminder or stimulus, because the child isn’t obsessed with a fear of being contaminated or of losing things or of causing an accident or injury. Some kids with OCD just feel compelled to do things a certain number of times, in a certain order, or in a balanced, symmetrical way. Similarly, some kids blink, tap objects, or make some other motion that has no real purpose over and over, for no particular reason except that OCD makes them do it.
